Nutrition Scheduling
Front desk hours:

  • Sessions are by Appointment Only, with the exception of Pilates Mat & Yoga
    classes.
  • All Pilates, Yoga, and Personal Training clients are required to have a health
    history and participation waiver on file.  
  • Prices are subject to change without notice.
  • Discount offers cannot be combined with any other special pricing offers.
  • Sessions or packages may not be refunded after one year of purchase, but the
    value may be transferred to another EVOLUTION service.

Cancellation & Registration Policies:
  • Pilates & Personal Training 24—Hour Cancellation Policy (effective August 1,
    2011)—All Private, Personal Training & Pilates Equipment class sessions
    require 24 hour notice of  cancellation or rescheduling to avoid being charged
    for that session.
  • Nutrition 24 - Hour Cancellation Policy (effective Nov. 1, 2009)  All Nutrition
    Appointments* require 24 hour notice of cancellation or rescheduling to avoid
    being charged for that session.
  • Registration for seminars, workshops, and courses is not complete or confirmed
    until payment is received.
  • Registration for Evolution (One hour) seminars is non-transferable / non-
    refundable.
  • Cancellation 2 weeks prior to course / workshop date may be refunded or put on
    account for other services / seminars.
  • Cancellation less than two weeks prior to course / workshop date is non-
    refundable, but may be transferred to other services or future course / workshop
    of choice.

Additional Policies that apply to the individual service:

Pilates / Personal Training:
  • First session for New Clients will include fitness evaluation.
  • Group Equipment Classes require pre-payment to secure a spot.
  • Group Equipment Classes require pre-registration to secure a spot.
  • All classes (Private, Duo, Grp. Reformer) are 50 min in length.  

Nutritional Services:
  • All new Evolution clients who will be using health insurance need to fill out our
    Health Insurance Registration form.
  • All new EVOLUTION clients must read and sign our Financial & Cancellation
    Policy.
  • Evolution accepts medical insurance plans, however it is the responsibility of
    the client to check on insurance coverage.   
  • The client is responsible for payment of all nutritional services.
  • Nutrition 24 hour cancellation (effective Nov. 1, 2009):
- All appointments* require 24 hour notice of cancellation before the
scheduled time to avoid being penalized.
- If notice of cancellation is given 24 hours prior to the scheduled
appointment, or you fail to appear for any appointment:
- Nutrition clients who use our 5-visit card will forfeit one visit.  If
no visits remain on the card, a $20 fee will be charged to
your account.
- Those who pay through their health insurance, or who pay by
any other method, will be charged a $20 fee.  
Note that this fee
will NOT be charged to your insurance company, but charged
directly to you.
- If you are 15 minutes late or more, to your appointment we reserve
the right to reschedule your appointment.   If we unable to see you
on the day that your appointment has been scheduled, due to a late arrival,
the appointment will be considered missed, and the fees described above
will be applied.   If you expect to arrive fifteen minutes late or later, for your
scheduled appointment, please contact us as soon as possible so that we
can make our best efforts to accommodate you.
- You are personally responsible for all fees accrued due to late cancellation
and/or failing to show up for a scheduled appointment.

*Nutrition members who pay a monthly membership fee will not be charged
for any cancelled or missed appointments.

EVOLUTION pilates & nutrition is in compliance with the Health Insurance Portability
& Accountability Act, protecting  an individual's health and personal information.
